          a.  MINORS MUST SUBMIT THEIR BIRTH CERTIFICATE (including renewal) *Birth Certificates

               for ALL applicants, regardless of age, MUST include the parent(s) names if applicable. 

               Children who have a previously issued passport MUST submit both the passport and 

               Birth Certificate.

5.  PROOF OF IDENTITY Original Driver's License, State Issued ID, or Military ID (If you are from a State other than Florida additional identification will be required)

6.  BOTH PARENTS & CHILD(REN) MUST BE PRESENT TO APPLY FOR A MINOR AGES 15 AND UNDER

7.  Children 15 and younger must have consent of both parents - Please ask Clerk about parental consent options

          a.  Please note: Parental Consent forms may not contain any errors or white-out.  Forms

               MUST BE completed in their entirety before given to the Clerk.  Any errors or missing

               information will make the form VOID.

 

Both parents must be present with valid Government issued Photo ID or One parent or legal guardian with one of the following:

• Form DS-3053: Statement of Consent Copy along with a copy of the front & back of the identification/drivers license
• Form DS-5525: Statement of Exigent/Special Family Circumstances
• Birth certificate naming only one parent
• Certified Court Order
